Benefits for Your Practice
Join a growing network of providers serving rural New York communities.
Reach Underserved Patients
Connect with community members who have been unable to access care due to geographic, financial, or technological barriers.
Reduce No-Shows
Our navigators prepare patients for visits — confirming appointments, arranging transportation, and ensuring technology readiness for telehealth.
Bilingual Patient Prep
We prepare English and Spanish-speaking patients before they see you, reducing language barriers and improving visit quality.
Zero Integration Required
We don't touch your EHR or systems. Patients connect through YOUR existing telehealth platform. We just help them get there.
Save Staff Time
Our navigators handle benefits enrollment, insurance questions, and appointment logistics — tasks that often fall on your front desk.
Community Health Data
Receive de-identified, aggregate data about access patterns in your service area through our CB-CAP platform (opt-in).
How the Partnership Works
Join Our Network
Express interest through our foundation website. We review your services and coverage area.
We Prepare Patients
Our Health Navigators help community members understand their options and prepare for visits with your practice.
Patients Connect on Your Platform
Prepared patients connect to your existing telehealth system or schedule in-person visits through your normal process.
Feedback Loop
We gather anonymized feedback to improve the referral process. You maintain full control of the patient relationship.
You Stay in Control
SozoRock Health never provides clinical care, never accesses your systems, and never owns the patient relationship. We are a bridge — connecting prepared patients to your practice through your existing workflows.
Provider FAQ
No. There is no cost to join the SozoRock Health provider network. Our services are funded through grants and donations to The SozoRock Foundation.
No. We are platform-agnostic. Patients connect through whatever telehealth system you already use — Doxy.me, Zoom, MyChart, etc. We simply prepare them to use it.
We share only what the patient explicitly consents to: their first name, preferred language, and the services they need. We never share health conditions, insurance details, or Social Security numbers because we don't collect them.
Our Health Navigators help patients: set up the device, test audio/video, understand what to expect during the visit, and write down questions for their provider. This significantly reduces technical issues during visits.
Absolutely. If you have patients who need help with benefits enrollment, insurance navigation, or technology support, you can refer them to us. We handle the social determinants of health so you can focus on clinical care.
