Frequently Asked Questions
Common questions about Community-Based Health Equity Hubs.
How do I know when navigators will be in my community?
Register on our website to receive email or text notifications about upcoming events in your county. We also post schedules on community bulletin boards and social media.
Is Health Access Day only for uninsured people?
No! Health Access Day is for everyone. Whether you have insurance, need insurance, want to learn about benefits, or just want a free health screening, all community members are welcome.
Do I need to bring anything to a community event?
Bring any documents related to what you need help with - insurance cards, bills, ID, proof of income for benefits. If you do not have everything, come anyway. We can help you figure out what you need.
Are these events really free?
Yes, completely free. No registration fees, no service charges, no hidden costs. Our services are funded through grants and donations.
Can I bring my family?
Absolutely! We encourage families to come together. We can help with insurance for children, benefits for families, and health resources for all ages.
What languages are available?
Services are available in English and Spanish. For other languages, we can arrange interpreters at no cost with advance notice.
Can you help with private insurance from my employer?
Yes! We can help you understand your employer insurance, compare it with marketplace options, and connect you with benefits that supplement your coverage.
Do you provide medical care at these events?
Community-Based Hubs provide health screenings and wellness checks, but not medical treatment. Health Access Day may include clinical services like vaccines through partnerships with local health departments. We always connect you to appropriate care.
How can my organization host a community hub?
We partner with faith centers, community organizations, and local groups. Contact us through the website or call The SozoRock Foundation to discuss hosting opportunities.
What happens to my information?
We collect only basic contact information to follow up on your needs. We do not collect medical records or diagnoses. All information is confidential. See our Privacy Policy for complete details.
