Frequently Asked Questions

Common questions about the Home-Based Health Equity Hub program.

How long can I keep the tablet?

As long as you need it. There is no time limit. When your situation improves, you recover, regain mobility, or no longer need it, simply return it using the prepaid shipping label. If your needs change to long-term, we can discuss transitioning to permanent ownership.

What if I am not tech-savvy?

The tablet is designed for people with no tech experience. Your navigator provides in-home training and is available by phone to walk you through anything. Large buttons, simple menus, and voice commands make it easy to use.

Do I need Wi-Fi or internet at home?

No! The tablet has built-in cellular service like a phone. It works anywhere with cell coverage, which includes most of rural New York. No Wi-Fi setup, no internet bills, no tech hassles.

Can family members use the tablet too?

Yes, for health purposes. If you are caring for a spouse, parent, or child, the whole household can use it for telehealth appointments and health management. We can set up multiple user profiles.

What if the tablet breaks or gets damaged?

Accidents happen. Normal wear and tear is expected and covered. If the tablet is damaged, let your navigator know immediately. We will assess the situation and provide a replacement if needed at no cost to you.

Is there really no cost?

Really no cost. No security deposit, no monthly fees, no hidden charges. This program is funded by grants and donations to The SozoRock Foundation. If we ask you to return it and you cannot afford shipping, we will send a prepaid label.

Can I use the tablet for non-health purposes?

The tablet is primarily for health use, but we understand people need to check email, video call family, or look things up. Reasonable personal use is okay. We block inappropriate content and monitor data usage to ensure the cellular plan stays affordable.

How do I apply?

Click the Apply Now button on this page, or call The SozoRock Foundation. Be ready to explain your situation, why you need home-based support, and how a tablet would help you access healthcare. We do not require proof of income or medical documentation for the application.

What if I do not qualify or the program is full?

If we cannot serve you right away, we will put you on a waiting list and connect you with alternative resources. We are expanding this program as funding allows, so needs today may be met in the future.

What telehealth services can I access?

The tablet works with any provider offering telehealth. We pre-install common platforms like Zoom, Doxy.me, and major hospital patient portals. Your navigator helps you connect with your specific providers and set up accounts.

Early Access Program

The Home-Based Hub is currently in early access with limited availability. We are prioritizing applicants with the most critical needs and will expand the program as funding permits.

Even if we cannot serve you immediately, please apply. We will keep your application on file and notify you when tablets become available. In the meantime, we will connect you with alternative resources.

Ready to Apply?

If you or someone you care for is homebound and needs better access to healthcare, apply today. The application takes less than 10 minutes.
